Caregiver requirements
- 18 years or older with a valid Social Security number
- Able to pass a criminal background check
- Reliable, patient, and compassionate
- Family members and friends of the client are encouraged to apply
- No prior professional experience required — we provide training through your caregiver portal
After you apply
You'll get access to the Caregiver Portal, where you can upload your ID and documents, enter your CHAMPS Provider ID, complete onboarding forms and training, see your assigned clients, and track credential expiration dates — all from your phone.

- Complete the application — it covers the MDHHS registry form DCH-1421 and your CHAMPS enrollment details.
- Pick a time for your phone interview.
- Once hired, the same login opens your new hire forms and your training.
